      I downloaded the ISO last year but in view of the issues never installed it. Now seems a good time but I am unclear whether Microsoft update their ISOs during the life of a release or just rely on the post installation update system to apply all changes and fixes.

      If you are using the Media Creation Tool, Create Windows 10 installation media, download it, save it, then right click the .exe file and check the version.

      I just downloaded it and the version is 10.0.18362.1. So that is the original version 1903 release.

      Yes, they do.
      You can download MediaCreationTool1809.exe and get an updated 1809 ISO.
