I am on Word 2003 and I have developed a word form for our performance review document. I have to password protect the form so that users can only enter data into the fields I created on the form. But, when I protect the document the spelling and grammar functionality is grayed out. Can someone tell me how I can add a spell checker onto my form? I attached a sample copy of the form. The password is test.

Any help would be much appreciated. Thank you in advance.

I tested your doc in Word 2007. The Spelling & Grammar check is simply unavailable to the protected document. However, in the Editing restrictions section, if you allow Comments, the spell checker is available but you lose the form filling functionality.

Word forms cannot be spell checked WHILE they are protected for forms so you need to unprotect, spell check then reprotect. The easiest way to do this is by using a macro. The best source to implementing a solution would be on the Word MVPs site. You will also find sample code here but you should start with that webpage.
