Designing Form (Outlook XP)

Does anyone know where I can get instructions on creating a form (which will be a survey) in Outlook where people will be required to reply and fill-in and send back to me. I need to keep this form as simple as I can. I want to have a survey (form) where I will send to people, I want them to reply and fill by selecting from combo box, optionbutton etc (I know how to create these). What I am not sure about is:- when I send the form to people I want them to see the survey which they are required to make selections from the combo box, checkboxes, etc. When I received their results I want to see only the person

Try this link: http://www.outlookcode.com/d/forms/survey.htm%5B/url%5D. You’ll probably have to do a bit of tweaking, but hopefully it will give you a starting point.
