Doesn’t help. Every link from the first answer leads to the same page, the main Office Online template page, and not to any yellow notepad template.
Have you tried just Googling for images of notepads? I found plenty of them, many of which didn’t have copyright restrictions. If you’re graphically inclined, you can create a page-shaped image with a yellow background color and a horizontal and vertical few lines. You could even scan a piece of yellow notepad paper.
Once you have an image (and they really seem to be everywhere), insert into header. Set wrapping option to behind text, stretch to all page edges, and close the header.

Every Word document involves a template.
Are we talking Word here? or the NotePad Program?
There are not such options for the NotePad Program; that is plain text.
Word for the Mac has a Notebook view not in Word for Windows.

There is an easy method that makes each page look exactly like a yellow notepad sheet.
Step 1: Make a color scan of a blank yellow notepad page and save as a .jpg photo.
Step 2: Open a new Word Document.
Step 3: Select the Design tab in the Ribbon above, select the Watermark down arrow and select Custom Watermark to open Printed Watermark window.
Step 4: Select Picture Watermark. Click Select picture button to open an Explorer window to locate and select the .jpg photo you made from the yellow notepad sheet. Click the dropdown arrow and set the scale to 100%. Uncheck Washout if that is checked.
Step 5: Click Apply and Close the window.
Step 6: Save as a template for future use.
The Watermark method negates the need for adjusting an inserted photo or other formatting problems and simply replaces the background with an image ready to write on top of.

I downloaded the document from the link above and clicked on it to allow editing.
I’ve found that ‘exact’ line spacing at 25pt (0 pt before and 0 pt after) hits every scored line exactly, no matter the font or font size. 2.5″ left margin starts the typed line just past the red vertical scored line and .25″ right margin keeps it within right edge. Top margin at 1.77″ starts first line and bottom margin at .17″ finishes page on penultimate scored line.
I’m not sure what those dots are (circled in red). They seem to be in the template, but I can’t get rid of them.

Those dots were on the original notepad sheet .jpg that I scanned. I did it quick to demonstrate how to do it and didn’t inspect the photo. Appears that my scanner glass needed cleaning. Redid it and here the same .docx file without the stains. The real thing is you know how to do this for yourself.
