    I am desparately trying to create a form which I can send (only) to other departments for information only. No reply required. I thought I had it all figured out pretty well, until I tried sending myself the message…came through as blank.

    I would appreciate any help. I have attached my .oft file.

      I have just one more question…I have two radio buttons on this form. They work just find when I am filling in the fields…When the form is received neither radio button shows that it was selected.

      Is there something I should be doing in properties or something to make this happen. I have tried several things and it still receives with NO radio button selected.

      HELP please…
