RibbonCustomizer comes in a free starter edition (pschmid.net – RibbonCustomizer™ Starter Edition) and a commercial professional edition.

As far as I can see-the really hard work- writing code or macros for custom buttons it does not do. All it does is allow you to move around the existing MS buttons and even create new groups but only based on those.

You should read the three part article at Customizing the 2007 Office Fluent Ribbon for Developers (Part 1 of 3). Also, Jensen Harris: An Office User Interface Blog : Licensing the 2007 Microsoft Office User Interface if you plan to produce your own product.
