Words Sporadic Span Tags?

In Word, sometimes a sequence of words or even letters gets a Span tag inserted. While this isn’t noticeable in Word itself, it becomes an annoyance when exporting to Flare, for example. The span tags are in Word’s XML.

How do they get entered, and how can I prevent that?
