      Thanks!! The DateSerial seems to work however I would prefer to see the column heading in the format of Apr 2005 instead of 4/1/2005. How do I accomplish?

      Thanks! However, when I try this the counts in my individidual columns disappear and the header still does not indicate the year (i.e., Jan 2006, Feb 2006, etc.

      No, that didn’t fix the problem. Even when trying to print the calendar I get the message: Publisher cannot find the linked picture. I also get the options to either 1) Find the linked picture 2) Print displayed picture or 3) Print No Picture.

      I don’t have Acrobat other than being able to read an Acrobat docuemnt. So, I’m stuck with Word. The row can grow happily as the documentation grows. I don’t know whether to develop this document with one row or several rows. However, if I develop with one row when the form is locked the user won’t be able to add additional rows as needed without coming back to me.

      But in my Excel spreadsheet I don’t have a unique identifier column. And in Visio I don’t see where it asked me to indicate a unique identifier column.

      Thanks, again it works like a charm. However, when there could be multiple items selected from the list of items. How best should I handle this and still allow the user the capability to add an item that is not in the list of pre-defined choices?

      Thanks HanV! I added a combo list box to my form but how do I add my items. The VBA screen doesn’t look lke any of the Access screens that I’m familar.

      The forms are reproduced elsewhere. The form is not a merged document wherein I could simply have the page number printed on it where the page number would be known. Instead of having someone numbering from 1 to 1500 or more. I thought once I received the 1500 or more forms from the printshop that I could insert these forms into a local printer and somehow have a formula/macro or something to allow me to number the pages from 1 to X.
